Chemical dependency/ drug abuse evaluations are a free service offered by FAIR, and can be made during daytime or evening hours by calling (847)-359-5192 or completing an on-line evaluation request.
Referrals to treatment are made based on the evaluation results. These recommendations reflect the most appropriate level of care for each individual.
We ask that both a parent and the adolescent be present for this assessment, which takes approximately one hour. The bulk of this time is spent with the adolescent to obtain information concerning all areas of his or her life, especially information about alcohol and drug use. Sometimes a urine screen is helpful to verify recent drug use.
At the time of the evaluation we will ask for a urine toxicology screen from the adolescent to help verify recent drug use. The charge for this is $75. See our FAQ pages if you have other questions.
